To request a Transfer, the following criteria must be met:
· Request must be made by a current Advocate.
· The recipient must be anAdvocatein good standing with Debt Cleanse Group Legal Services, LLC. (“Debt Cleanse” or “Company”).
· Debt Cleansemust approve the proposed request.
Any requests submitted outside of the above guidelines will be considered on a case-by-case basis at the discretionof the Company.
I, ______________________________, the owner of _____________________ do hereby authorize this transfer request as outlined in the Debt CleansePolicies and Procedures. I fully understand that this request will forever relieve me of all ownership rights to the above referenced position. Ihereby release Debt Cleansefrom any potential or future liability or obligation caused by the following change.
